Section 560-X-51-.08 - Change Of Hospice(1) An individual or representative may change, once in each election period, the designation of the particular hospice from which hospice care will be received. The change of the designated hospice is not a revocation of the election for the period in which it is made.(2) To change the designation of hospice provider, the individual or representative must file a signed statement that includes the following information: (a) The name of the hospice from which care has been received.(b) The name of the hospice from which the individual plans to receive care.(c) The effective date of the change. A copy of this statement must be provided to the hospice provider and to the Alabama Medicaid Agency.
(3) The waiver of rights as addressed in Rule 560 -X-51-.06 remains in effect.Ala. Admin. Code r. 560-X-51-.08
