Section 545-X-2-.05 - Replacement/New License Certificate

A licensee whose license has been lost or destroyed may make application to the Commission for a replacement license certificate. Such application shall be accompanied by an affidavit setting out the facts concerning the loss or destruction of the license. Any licensee whose name is changed by marriage or court order may surrender his or her license certificate and apply to the Commission for a new license certificate. Such application shall be accompanied by a copy of the court order or marriage certificate. The fee for any replacement/new license certificate shall be $25.00, payable to the Medical Licensure Commission.
