Section 1647.6 - Physical evaluation and medical history; records(a) A physical evaluation and medical history shall be taken before the administration of moderate sedation.(b) Any dentist holding a permit shall maintain records of the physical evaluation, medical history, and moderate sedation procedures used as required by board regulations.Ca.
