Section R. 432.21721 - Financial statement requirementsRule 721.
(1) A numeral game licensee shall submit a financial statement to the bureau on a form, provided by or approved in writing by the bureau, by the tenth day of the month following the month that the licensed gaming event was held.(2) Millionaire party and large raffle licensees shall report all numeral game ticket revenue, merchandise prizes, and purchases on a form provided by or approved in writing by the bureau.(3) The financial statement shall be signed by the principal officer of the qualified organization certifying that the information on the financial statement is true, correct, and complete to the best of his or her knowledge.Mich.
